While nurses were long required to wear special uniforms, surgeons and physicians did not wear any specialized discount scrub uniforms or garments until well into the twentieth century. Surgeons performed procedures in an operating theater which often had an audience of medical students and nurses. The surgeon was dressed in his usual street clothing, perhaps with a butcher's apron to keep bloodstains from his clothes. The surgeon operated with bare hands and non-sterile supplies and instruments. Silk and gut sutures were sold in open lengths, and reusable hand-threaded sewing needles were used for suturing. Packing gauze was simply made from the sweepings taken from cotton mill floors.
As a contrast to the cleanliness and sterility which mark the surgical environment and medical apparel such as discount medical scrubs today, a century ago the mark of a surgeon was the profusion of fluids and blood on his clothing. The rise of the antiseptic theory first promulgated by Lister, in conjunction with the pandemic of Spanish flu in 1918, led some surgeons and medical attendants to wear gauze masks in surgery. This was not done to protect the patient from infection, but rather to protect surgeons and attendants from the patients' diseases. At this time also, medical staff in the operating theater began to wear heavy rubber gloves in order to protect their hands from the strong disinfectant solutions which were used to clean the equipment and room. Surgeons soon adopted this practice as well.
By the Second World War aseptic technique and the science of preventing infection of wounds led to the adoption of gowns and antiseptic drapes for operating room work. Supplies, instruments and dressings were sterilized routinely either by using ethylene oxide gas or by high pressure steam. Originally operating room apparel was made of white fabric in order to emphasize cleanliness and sterility. However the bright operating lights shining on an all white environment led to severe eyestrain for many surgeons and medical staff. As a result, in the twenty years after World War II most hospitals had foregone white operating room attire in favor of shades of green or blue, which is more restful to the eyes and makes bright red blood blotches less noticeable. By the nineteen seventies surgical and medical cheap scrub sets had reached their current state: normally a short sleeved V neck shirt with drawstring pants' or a short sleeved calf length dress, made of green cotton or polyester fabric. Often this simple uniform was accompanied by a bouffant style cloth cap, a surgical gown, latex gloves and gauze mask, and closed toe shoes.
Originally this uniform was called "surgical greens" because of the color, but soon came to be called "discount medical scrubs" because it was used in a scrubbed environment. At present, any discount scrub uniforms which consist of a short sleeve shirt with pants are called scrubs. Practically all medical care personnel in the U.S. and Canada wear some form of cheap scrub sets, as do workers in medical and dental offices.
