Surgical scrubs or any kind of specialized garments for surgical procedures came into use only after the early 20th century. Earlier, surgeons most often wore their street clothes with butcher's apron to guard their clothes from blood stains.

Unlike the modern times when the values of cleanliness and conscientiousness are attached to the medical profession, in the past, the mark of a successful doctor was the profusion of blood on his clothing. The transmission of diseases and infection was a controversial subject and the necessity of uniforms as emblematic of a particular occupation was greatly debated within the profession.

The 'Spanish Flu' pandemic in 1918 and the growing awareness of Lister's antiseptic theory led some surgeons to wear cotton gauge masks while conducting surgeries. At around the same period, operation theatre staff took to wearing heavy rubber gloves while cleaning the operation room. Both, the mask and the gloves, were worn not for the patient's wellbeing but for protecting the surgeon and the staff from being infected with the patient's diseases.

The 1940s witnessed advances in the aseptic technique as well as science of injury infection which encouraged the use of antiseptic gowns in the OR. White was chosen as the OR attire color to emphasize on cleanliness. However, bright lights in the operation theatre combined with an all-white environment, caused eye fatigue. By the '50s and '60s, the white apparel was abandoned in favor of different shades of green. Green created a high-contrast surrounding, reduced eyestrain and made blood blotches less conspicuous.

By the 1970s, surgical clothing had almost reached its current state. A green short sleeve V-necked shirt was worn along with drawstring pants of the same color, or a calf-length dress. This was accompanied with a bouffant-style cap, a synthetic textile mask and surgical gown. Latex gloves and closed-toe shoes completed the outfit. This uniform was initially called 'surgical greens' but later was termed 'surgical scrubs' since it was to be worn in a 'scrubbed' atmosphere.
